Clinical Experiences in Total Correction of Unilateral Cleft Lip Nose Deformity

Abstract
Numerous methods for correction of secondary cleft lip nose deformity have been developed and new methods are introducing every year, but there was no standardized procedure with satisfactory results. Although the philtrum plays a keyrole in the appearance of the upper lip and nostril sill, the formation of philtrum have been resulted in a unsatisfactory aesthetic outcome. The author performed 19 cases of the correction of unilateral cleft lip nose deformity between may 1993 and May 1996. The pathologic deformity of the patients was divided into 4 groups, paranasal depression due to maxillary hypoplasia, residual nasal deformity, secondary lip deformity and loss of anatomical structure. The author augmented paranasal depression with silastic block and autogenous conchal cartilages and performed alar cartilage reposition using open rhinoplasty and secondary cheiloplasty incision. And autogenous palmaris longus tendon was used always for reconstruction of pohiltral column. We experienced sattisfactory results in 19 patients who had secondary unilateral unilateral cleft lip nose deformity. In conclusion, this study shows that total correction including alar cartilage reposition, secondary cheiloplasty, and paranasal augmentation obtained excellent results than partial correction or staged correction.
